What is All-on-X?
All-on-X is a full set of upper or lower teeth attached to four to six dental implants. The implants act as roots, and the teeth are one connected bridge that screws onto them, so they don't come out and they don't move. You may have heard it called All-on-4, All-on-6, teeth in a day, full-mouth dental implants or permanent dentures. They all describe the same idea. The "X" is simply the number of implants your jaw needs.
The back implants are usually placed at an angle. That lets them reach the strongest part of your jaw and stay clear of the sinus in the upper jaw and the main nerve in the lower jaw. It's the reason many people who were told they don't have enough bone can still have All-on-X, often without a bone graft.
- Your new teeth: one fixed bridge, screwed onto the implants. Only we take it off.
- Front implants go straight in, where the bone is usually thickest.
- Back implants are angled to reach strong bone and avoid the nerve or sinus.
- The nerve in the lower jaw. In the upper jaw, the sinus plays the same role.
Is All-on-X right for you?
It's usually a good fit if one of these sounds like you:
- Most of your teeth are broken, decayed, loose or failing
- Gum disease has loosened your teeth and you've been told some need to come out
- You wear a full denture and you're tired of it slipping, making you gag, or needing adhesive
- You want teeth that don't come out and don't cover the roof of your mouth
If you still have some healthy teeth, All-on-X may be more than you need. Individual implants, a bridge or a snap-in denture might fit better, and we'll tell you if so.
How it works, step by step
- Day oneFree consult and 3D scan
We talk about what's been going on with your teeth, take a 3D CT scan, and look at it together on the screen. You'll leave with a written plan and a price.
- 1 to 3 weeksDesigning your new teeth
We take digital scans, photos and bite records, and design your teeth: the shape, the shade, and how your smile sits with your face and lips.
- One visit per archSurgery day
Any remaining teeth on that arch come out, the implants go in where we planned them on your scan, and in most cases your fixed temporary teeth are attached before you go home.
- 3 to 6 monthsHealing
The implants fuse with your jaw. You'll eat softer foods and come in for a few short check-ups.
- After healingYour final teeth
We make your permanent bridge in zirconia or in acrylic on a titanium frame, try it in, adjust it, and fix it in place.
- OngoingKeeping it healthy
Cleanings a few times a year, a water flosser at home, and we check the implants and bridge at every visit.
What surgery day is like
This is the day most people are nervous about, so here's how it usually goes.
If you've chosen oral sedation, you'll take a single dose of anti-anxiety medication before you arrive, and someone will need to drive you. We numb everything thoroughly, and you can have nitrous oxide during the procedure too.
Dr. Ansari removes any remaining teeth on that arch, smooths the bone, and places the implants where they were planned on your scan. If the implants are stable enough, which is usually the case, your temporary teeth, made ahead of time from your design, are attached the same day and you go home with fixed teeth. If they aren't, he'll explain the plan for getting your teeth on safely.
Expect swelling and soreness for a few days. You'll leave with written instructions and a number to call, and we'll see you back soon after to check how you're healing.
Your final teeth: zirconia or hybrid
Once the implants have healed, we make your permanent bridge. You choose the material.

Compare your options
All-on-X vs. dentures, snap-in dentures and implants
There's no single right answer. Here's how the main ways to replace a full arch of teeth compare.
| Feature | Regular denture | Snap-in denture | All-on-X | Individual implants |
|---|---|---|---|---|
| How it stays in | Suction and adhesive | Snaps onto 2 to 4 implants | Screwed onto 4 to 6 implants | Each tooth or bridge on its own implants |
| Do you take it out? | Yes, every day | Yes, every day | No. Only we remove it | No |
| Covers the roof of your mouth? | Yes (upper) | Often (upper) | No | No |
| Chewing | Weakest | Much better than a denture | Close to natural teeth | Close to natural teeth |
| Jawbone over time | Keeps shrinking | Shrinks less near the implants | Shrinks less | Shrinks less |
| Implants per arch | None | 2 to 4 | 4 to 6 | Often 6 to 10 |
| Relative cost | $ | $$ | $$$ | $$$$ |
| Best for | Tight budgets, or while planning implants | Denture wearers who want it to stay put | Failing teeth, or denture wearers who want fixed teeth | A few missing teeth with healthy bone |
Not sure which fits you? That's what the free consult is for. We'll show you each option on your own scan.

Questions
All-on-X questions we hear a lot
How much does All-on-X cost?
Hybrid final teeth (acrylic on a titanium frame) start at $18,995 per arch. Zirconia is an upgrade, at $22,000 to $24,000 per arch. The price includes your consult and plan, removing any remaining teeth on that arch, four to six implants and their parts, nitrous or oral sedation on surgery day, temporary teeth, your final teeth and check-ups while you heal. If you need a bone graft, we'll tell you the cost before you decide. You'll get your exact price in writing at the free consult.
Does All-on-X hurt?
You're numb for the surgery, and you can add nitrous oxide or a single dose of anti-anxiety medication beforehand. Afterward, most people are sore and swollen for three to five days, about like having several teeth pulled, and they manage it with the pain medicine we recommend.
How long do All-on-X teeth last?
The implants themselves can last decades with good care. The teeth on top wear over time. Acrylic hybrid teeth often need refreshing or replacing after five to ten years, and zirconia usually lasts longer. Regular cleanings, and a night guard if you grind, make a big difference.
I was told I don't have enough bone. Can I still get it?
Often, yes. Angling the back implants lets us use the strongest bone and stay clear of the sinus and nerve, which is one reason All-on-X often works without bone grafting. The CT scan at your consult tells us for sure.
Can I eat normally?
After healing, yes. Most people are back to steak, salad, apples and corn on the cob. For the first few months, while the implants fuse to the bone, you'll stick to softer foods.
Is it permanent? Can I take it out?
You can't take it out. The bridge is screwed onto the implants, and only we remove it, usually to clean underneath. That's the main difference from a regular or snap-in denture.
How do I clean it?
Brush it like teeth, and use a water flosser under the bridge every day. We'll show you how. You'll also come in for professional cleanings a few times a year.
What's the difference between All-on-4 and All-on-X?
All-on-4 is a brand name for a full-arch bridge on four implants. All-on-X is the general term, where X is the number of implants your bone and bite call for, usually four to six.
Do all my teeth have to come out?
Only on the arch we're treating. All-on-X can be done on the top, the bottom, or both. If some of your teeth are healthy, there may be a better option, like individual implants or a partial, and we'll tell you.
How much time off work will I need?
Most people take two to five days off after surgery. You'll have your temporary teeth, so you won't be out in public without teeth.
Does insurance cover All-on-X?
Dental insurance rarely covers the whole thing, but it often pays toward parts of it, like extractions, implants or the teeth, up to your yearly maximum. We'll check your benefits before you decide.
Am I too old for All-on-X?
There's no upper age limit. Your general health matters more than your age, and we'll go over your medical history at the consult.
What if I smoke or have diabetes?
Neither one automatically rules you out, but both slow healing and raise the risk of implant problems. Well-controlled diabetes is usually fine. If you smoke, cutting back or quitting around the surgery helps a lot. We'll talk about your situation at the consult.
